Health Records

All patients have the right to access their own health records. This page discusses how you can access your own Health Records, or information on behalf of a patient.

Personal Health Information Protection Act

The Personal Health Information Protection Act (PHIPA) grants patients the right to access their own health records. PHIPA also requires institutions to protect patient health records and keep them confidential.

If a patient or designate wishes to request personal health information specific to their Ornge transport, they may complete a Personal Health Information Request Form.

Personal Health Information Request Process

Are you interested in accessing your Personal Health Records, please follow these steps:

  1. Complete the Personal Health Information Form

  2. Write a cheque for $30.00 payable to Ornge (for more information about fees please visit our Privacy and Information Page)

  3. If you are acting on behalf of the patient, include any documents necessary to prove that you have the power to do so, e.g. power of attorney, will, etc.

  4. Mail the completed forms and documentation (steps 1 -3 ) to the address below:
